Thank you for trusting us with your care. To enhance patient services, South Georgia North Florida Eye Partners plans to close our Madison office (located at 234 SW Range Avenue, Madison, FL 32340), effective December 30, 2025. While this was a difficult decision, it allows us to further strengthen care across our practice. We remain fully committed to serving you and welcome you to visit any of our other four locations.

4. What happens to my scheduled appointment?
If you have an appointment scheduled at the Madison office after the closure date, our staff will reach out to you to reschedule at one of our other locations listed above. If you have not been contacted, please call us at: 229-244-2068 for assistance.

Your medical records will remain securely with South Georgia North Florida Eye Partners. If you would like to transfer your records to another provider, you can request a records transfer by completing a form, which is available at the front desk at any of our locations.
